High-deductible UHC plans, plus Kaiser. The premiums are fairly high. If an employee is on unpaid leave (maternity, disability, etc - which is easy with 3 weeks parental leave), the premiums are invoiced to that employee.

For the person who never needs to use it, it's IDEAL! the cheapest health insurance I've ever had. And you can invest what you would have normally paid for it, in your account (HSA) that is your untaxed money. HOWEVER, the insurance doesn't do you any favors until you hit a minimum amount (Usually in the thousands, but it's a savings account of your own money). So, if you are a person who needs frequent medical assistance, or have a family than it might not be the best at first.
