This self-funded plan is managed by Health Smart utilizing Cigna’s PPO National Provider Network. There are two different plans with low and high options available.

This benefit is available to all full-time benefit eligible employees. Employees may request dependent coverage.

Low (Deductible:$1,750)
High (Deductible:$750)

Both plans have the same co-pays for doctor office visits and prescription drugs.
