Vision Insurance

Vision coverage is offered by Davis Vision through Guardian Life. This plan will pay a greater benefit amount if an in-network provider is utilized. There is a $10 exam co-pay and a $25 materials co-pay. The plan pays for services utilizing the below schedule:

• Eye exam once every 12 months from last date of service.
• Lenses once every 12 months from last date of service.
• Frames once every 12 months from last date of service.
• Contact lenses once every 12 months from last date of service. If contact lenses are chosen, participant isn’t eligible for frames for 24 months or lenses for
  12 months.
