Medical coverage is offered through Cigna for PPO and a High Deductible plan with the options of FSA or HSA, if you have a high deductible plan. Employee Assistance Program is offered by Canopy or Guardian, I can't remember which it is that we're switching to for EAP benefits. Our life insurance coverage dropped significantly to only covering $15,000 on an employee with the option to purchase supplemental life insurance terms for you and your family. Vision and dental is no longer paid by the company and is something the Employee has to cover out of their pocket through Guardian.
