When plan changed to Cigna all my doctors were suddenly out of network. Cigna routinely denied my claims because I had not proved I didn't have a preexisting condition!

Maybe this is a comment more generally for the state of health insurance in this country (US), but it's complete garbage. I have the CIGNA HDHP, which offers an extremely high deductible and essentially no tangible benefit besides access to a tax deferred HSA. Soooo.... I pay for everything myself. Needless to say I don't go to the doctor, even when I am sick or injured. Something frivolous like physical therapy (which I need and have been referred for) has been indefinitely deferred.
The VSP plan is pretty solid, however.
