BCBS is a MAJOR pain to work with. I had a procedure that should be covered at 100% under federal ACA guidelines. I verified this with my insurance prior to the procedure.
I've now been fighting with them for months as they conveniently do not have federal ACA guidelines or laws built into their system, so it's being billed against my deductible (which is incorrect and probably ACA noncompliant/against the law). I have had to provide links and paperwork and documentation, and have spent over 30 hours trying to educate BCBS on this and they do not understand.

Costs across the country are the issue here, not a NTST specific issue. They do try to offset increases year over year, but it is still a high premium. Coverage seems to be good for everything we have needed outside of hearing aids.
