Supplemental Life Insurance

All full-time (30+ hours) are eligible to enroll in employee-paid voluntary life and AD&D insurance on or after the 1st of the month following the date of hire. Employees may be required to provide evidence of insurability for amounts over the guaranteed issue amount. Your combined Basic Life and Voluntary Life amounts cannot exceed a maximum of 6 times your annual earnings up to $2 million dollars. The policy is portable and convertible at different rates.
