Surprising things are not covered. My little one has a speech impediment. They denied us because he was not mentally incapacitated. The person on the phone was almost gleeful in their denial of this coverage.

Originally very expensive and limited options were offered. Management later upgraded the plans to make them more competitive. The newer plans were acceptable.

It is great because it gives good discounts on doctor visits, just don't cover everything which you can expect that from most insurance companies
