3. was told to expect a "mock patient encounter," followed by meeting with co-founders, but never heard from them (and I didn't follow up since I accepted another offer)

Very lengthly onboarding process where communication was only ever though email, however, they are not prompt on responding. There was one brief zoom interaction that was a mock patient encounter. No curve balls, it was very straight forward. After onboarding for nearly 2 months and believing I would be starting later that week, they informed me of a new requirement- that I would need to be licensed with a DEA in an additional state.
