It really depends on you professor, you can either contact directly the professor you are interested in and ask if she/he is looking for a PhD student, get to be interviewed and if he is ok with you you will apply to the Phd programm she/he is affiliated to,
or you can apply directly to the Phd program, hope to pass the first selection based on cv and do go trought the hiring process which depends on the specific PHd programm you are appling to.

The questions really depend on the professor and on the subjust, udually they ask what did you do in your master, what are your research experinces and so on.
I didn't go through an interview. I did my master studies at EPFL and was offered a PhD student position by two different professors at the end of my master.

A 50-minute interview was conducted, during which my knowledge was tested. They asked exclusively about the technical design process, and the only other question was my motivation for applying to the PhD program.
