I applied online. The process took 2 weeks. I interviewed at Life Time (Minneapolis, MN) in Oct 2011
Interview
Pretty standard interview process. First interview counts and the second interview is more of a formality to make sure the GM thinks you're not a psycho. The name of the interview method evades me, but they follow that standard corporate method of asking situational and behavioral questions to see how quickly you'll respond. The person conducting the interview is given a list of say 30 of these questions and selects/prints off the questions they find most applicable.
For the second interview they'll ask you to memorize the Mission and Vision. I was e-mailed these exactly 30 minutes before the second interview (they're long and obnoxious) so luckily they didn't come up, but make sure you have them down.
Otherwise there is also expected inquiring about your job experience. If you can handle the behavioral questions it is pretty easy.

Question 1
What is something you sacrificed in order to work?
Quick conversion. Wasnt a formal interview. We exchanged a few questions about the position and was on my way. Was told they hada few other people to interview but judging by the informal.meeting I assumed they just said that.
Easy going and very comfortable The questions were simple and straight forward while also informative about the job. I will say it did not seem to correlate too much to operations specifically but the training made it all worth it. Very thorough and an enjoyable, simple and easy interview

Question 1
How have you helped others in the past and what were you most proud
The interview went great! The manager was amazing, and offered me the job even when I had no work experience. I gladly accepted, and had a good experience working at Lifetime Fitness
