I applied online. The process took 4 weeks. I interviewed at Portland State University (Portland, OR) in Jan 2013

Interview

Mostly based on the advisor I applied to. Sent in an application and was invited to fly in for an interview. The interview portion itself was pretty brief - maybe an hour or so with the primary advisor - but the entire process lasted a day and a half. I received an officer within a week of the visiting day.
