Too many conflicting priorities. Not enough resources but workload remains the same which results in overworked employees and burn out.

Too much focus on having fun on the clock with coworkers really leaves customers hanging. You can make or exceed all your metrics, have perfect attendance, talk in the team chats when appropriate, and still be called out for not being social enough. Many leads are just too immature and don't communicate effectively. Too many "mean girls".
