Virtually everyone has been dismisses across the board from executives on down. Blackbox bought out by an firm from India (AGC) They're trying to export all the jobs possible to India and replace field employees with contractors.

No repercussions for others not doing their job. No real standards or clear expectations. No regular communication from management. Job is constantly changing who is responsible for what.
