Little diversity in leadership, mostly white males still dominate the organization. Many of those in leadership positions within IT, and within the organization overall, have been with the organization for many years and ran out of fresh, innovative ideas decades ago. The lack of diverse representation translates to job opportunities and upward mobility - cronyism is very present. No clear paths for advancement and a general feeling of fatigue. There is a big push to talk about the "culture" of the organization, but that culture is largely a false bill of goods.

They are afraid to let management level people go, or they are not willing to take any actions to the management level people. even if they are the sole reason to make people leave, they still keep them, only because those managers/team lead are so outdated and they can't find jobs from other places, or simply because they are with the company over 10 years. Upper management is not realizing that people are the key to the company; if a whole team suddenly all left and they have told you why, and they still don't take any action, then the history will repeat.
