Beyond poor management, any good suggestions fall on deaf ears, if you are incompetent at your job they'll create one for you and promote you, if you worked with your manager at a previous job you get special treatment, safety isn't job number one, no ability for upward movement unless you complain all the time about your job and they'll put you in secret teams with more ability for growth, the older you are the less likely you'll keep your job through a restructuring of the company
