ChenMed’s full-risk, value-based care model has successfully reduced hospitalizations among our patients by 30-50%! We are proud that we get to see our patients as frequently as needed to keep them healthy, out of the hospital and in the community where they belong! Additionally, we take our responsibility to comply with health information privacy seriously. Part of that includes protecting equipment that contains confidential information. To do this, we must navigate a highly regulated space in order to keep our patients’ health records safe and sound.
