Multiple roled leadership and a lack of empowerment and accountability create incapable, and/or incompetent management while multiple programs/projects all begin to fail at once due to the pile of impossible last minute requirements and a lack of true oversight that all inevitably fail the customer and fail the local program governance (that could have mitigated the problems and program risks).
