Multiple deadlines pressures, micromanagement, and unrealistic expectation of constant perfection within a time constraint with multiple projects due. There’s not enough time to do everything perfectly the first time. Design is a iterative process. Also, an engineer is only as good as the info that’s given to him/her. The person who back-checks the work gets angry at the designer for forgetting a clause or an equipment. The culture makes people feel intimidated to make mistakes but that’s how everyone learns.
