- Lack of focus on strategy, seemingly endless shifts in priorities week to week, month to month, year after year. Not congruent change, not progressive change, but chaotic disorienting change.
- Upper management claims to have an absolute "No A-holes" policy, claims to want to encourage cooperation and growth. Then turns around and is fully complacent with obviously toxic behavior from people in positions of authority, especially technical authority. These toxic interactions gradually wore down people's desire to engage.
