IT using stone age tools in some areas, very slow to upgrade tools and hardware, Some management is very frustrating to deal with as they are never present and do not pay attention to what their own staff is doing on an operational basis. Everything is about getting the annual bonus. If it's not on someone's annual plan, it probably won't happen. Work/life balance does not exist in some parts of IT. Poor annual planning of initiatives followed by employees giving up vacation time, evenings and weekends.
