several levels of management and responsibility distribution dilute the decision making process, and make for poor understanding of roles and expertise that is allocated to a team or division. Often times when a problem is recognized, the answer is to throw EVEN MORE people at it, further exacerbating the largest problem facing the organization... it's bulk and inability to move at a pace conducive to changing with consumer expectations
