Pros

Fully remote work, supportive colleagues

Cons

They are expanding way too fast, such that current physicians are not able to fill up their caseload in the timeframe that was projected during recruitment, due to too many doctors and too few patients. They provide a guaranteed monthly bonus for 6 months while you ramp up, but even this does not feel like it will be sufficient, especially if you do not treat certain conditions like ADHD (which is the vast majority of the current patient base). Base salary is far below market value and should be increased if this many physicians are struggling to make the projected numbers for incentive compensation. There are efforts to get more patients, but support staff are stretched so thin that these patients are no longer being adequately screened and thus are often needing to be referred out because they are not appropriate for a telehealth level of care. There also does not seem to be a good plan for patients on controlled substances once the public health emergency ends and the Ryan Haight Act is reinstated. Benefits are being changed bit by bit (parental leave, insurance premiums not all being 100% covered, how PTO is used) which I imagine is also a consequence of the massive hiring and expansion efforts. Transparency is another big problem, even though it is one of their core values. This company could be so great and has so much potential, but leadership needs to think long and hard about its priorities if it wants to retain its great physicians, who are the foundation of this company.

Pros

The work is straightforward. The clients are mostly reasonable. I prescribe a lot of stimulants in states where I can prescribe them. The electronic medical record and the resources contained within make it easy enough to do it. A good job at this. They offer the use of an AI scribe to help with documentation. You can be involved as much or as little as you want to in activities beyond direct patient care. Talkiatry chooses to make its money through the use of the psychotherapy add-on code. You're incentivized to spend time talking with your patients, which seems like a win for everyone.

Cons

Figuring out how the performance portion of the pay structure works is challenging and takes several months. You also take a significant hit in taxes because of the the back that the performance portion of your salary is paid as a bonus which is taxed at 30%. This is higher than the base pay tax rate. I'm hopeful that I'll see the significant tax return next April.
