Pros

Wonderful colleagues, flexibility, reasonable expectations for new patients and follow ups

Cons

Recruitment is filled with misleading information and marketing lures in inexperienced young docs. No one makes the promised salary- many struggle make over 250k. Incentive pay only accrues once you reach a certain patient load or productivity number. Your reimbursement may be closer to 25% billing vs 75%> Any "paid" time off negatively impacts your incentive pay. Taking 5 days off may cost you 50% or more of your incentive pay. Admin support is barebones unpredictable. New patients are often inappropriate and there is no prescreening. The emr and video software are not well designed for psychiatry. Physicians are expected to handle all administrative tasks, writing letters, prior auths, managing patient list, fielding questions from all pts

Pros

The work is straightforward. The clients are mostly reasonable. I prescribe a lot of stimulants in states where I can prescribe them. The electronic medical record and the resources contained within make it easy enough to do it. A good job at this. They offer the use of an AI scribe to help with documentation. You can be involved as much or as little as you want to in activities beyond direct patient care. Talkiatry chooses to make its money through the use of the psychotherapy add-on code. You're incentivized to spend time talking with your patients, which seems like a win for everyone.

Cons

Figuring out how the performance portion of the pay structure works is challenging and takes several months. You also take a significant hit in taxes because of the the back that the performance portion of your salary is paid as a bonus which is taxed at 30%. This is higher than the base pay tax rate. I'm hopeful that I'll see the significant tax return next April.
