You're incentivized to spend time talking with your patients, which seems like a win for everyone.

Figuring out how the performance portion of the pay structure works is challenging and takes several months. You also take a significant hit in taxes because of the the back that the performance portion of your salary is paid as a bonus which is taxed at 30%. This is higher than the base pay tax rate. I'm hopeful that I'll see the significant tax return next April.
