Middle management simply took orders from the people above them who were far too removed from the day-to-day work of the employees running the core operations of the company to actually understand how to make their company run better. When the company was doing well, everyone assumed that nothing could be improved; when the company was struggling, blame was simply thrust from senior management down the chain to entry-level employees without anyone actually taking the time to examine the structural issues ultimately at fault. Unclear as to why this was the case; maybe they didn't care, maybe it was just easier. I'll give them the benefit of the doubt and suggest that they just lack a thorough understanding of what exactly is going on in the basic operations of the company and therefore aren't going to know how to make it better. It boils down to this: stakeholders have a minimally-viable company that keeps churning out profits simply due to inertia, proprietary data, and a decent sales team, not because anything is working well behind the scenes. The company could actually be great if the right people were running it, but the decision-makers completely lack the technological sophistication to realize the company's potential.
