Poor leadership, leaders are more focused on gaining clients than expanding the company. Most teams are over-tasked. Developed applications are buggy and unsupportable. Works on antiquated equipment/software (recently decommed their last Pentium 3 server) Most systems monitoring is done by email alerts which often break the email server (10,000+ emails a day to sift through). Problems and issues constantly go un-noticed due to the sheer volume of alerts and holes in monitoring. New hires are often thrown into the fire with poor training and nothing more than hands-on experience.
