* High staff turnover, including executive staff * Lack of clear team leads with ownership and accountability. * Atmosphere is disrespectful with emphasis on being loud and hero of the day. * New hires are helpless and flounder around without documentation and mentors. * Senior developers do not lead by example. * Not a fun place to work. * Lack of standardized configuration and test beds available to all developers. * Tightly coupled system is hard to test. * Lots of wasted cycles over infrastructure, network and connectivity issues. * Company has made several penny wise, pound foolish decisions on equipment for developers. * Lack of proper planning – PRD reviews, design discussions/reviews, code reviews and completion estimates from developers. * Lack of discipline in branching and code commits. * Lack of single standardized tool for full release visibility. * Lots of interrupts from production support. No clear escalation path. * Unrealistic release dates with no plan for QA. Developers expected to rubber stamp arbitrary estimates.
