Quality development + failure analysis departments hardly go beyond making daily headlines of "who screwed up this time," rather than assist in problem resolution. Unfortunately, these departments are the ones who have the loudest, and often unquestioned, voices. Woe to anyone who works in an actual process department. -Production philosophy is like making a wafer of microchips that costs thousands of dollars is akin to producing a Twinkee. Taking the extra 10 minutes to properly investigating a problem inline is frowned upon, when production quotas are at stake. This ties in well with the last point, because these events typically become the bigger problems that ignite these conflicts.
