Pros

The biggest and most effective institution ever created for furthering medical science, Roughly half of all advances in modern US medicine have either been made by NIH staff or funded by NIH grants.

Cons

Being slashed and burned indiscrimately, Administration's tactics are creating chaos and waste, not "efficiency," and lives are already being lost. With luck, there will be an opportunity to rebuild, but it will take years.
