My department head was a nightmare to work for. She is known throughout the college for being difficult to work with and being a bully. Poor senior management is the only explanation as to how someone like that could be with the college 20+ years.

Miscommunication Senior management changing directions on too regular of a basis. It may not seem like a big deal to management, but if you view it as they are the handle of the whip and the bottom level employees are the end of the whip--the changes are seriously amplified Too much emphasis on administrative paperwork that gets in the way of actually spending time with the students.
