Growing too fast - multiple acquisitions the last few years and no strong leadership for their integration. Not adapting fast enough. Reorganizations every few months. Firm leadership's retirements depend on successful PE recap, thus extreme pressure on short term revenues. Too much focus on lobbying, not enough on people doing the work.

Very poor leadership in IT. Lack of vision and direction with a reactionary setting. Favoritism and poor communication dominate. Too much focus on the minutiae to see the big picture and an aversion to making the big decisions.
