pretty standard, plan was based out of Ohio or somewhere so I had to upgrade to get better coverage in my market. Was a subset of United Healthcare

Very poor health insurance, especially with the headquarters being in Pittsburgh (home to two of the most highly rated health care providers in the nation, neither of which are an option)
