Horrible UHC cost me 20,000 in one year. High out of pocket m high deductibles and a refusal to pay. Lies about claims and continuous modifications to accounts, Avoid UHC at all cost.

If you have an HSA plan, they usually deposit money in it for you if you do certain task, ie work out.

Health, Vision, Dental are all covered. Many provider options to choose from. They also pay upto $750 for HSA account, if you show them progress on fitness etc. It's a do it and claim kind of payment. Or you get nothing.
