Family Medical Leave

I have had to utilize this twice. The first time my mom became ill and then within a month she passed away. It was so easy to have her physician fill out the paperwork and i submitted it and that was it. Then my husband had a series of strokes and a heart attack and its been a nightmare with a headache. The didn't receive the paperwork or couldn't find it. I never received a copy this time. I wasn't informed as to how many intermittent hours I was allowed and there fore took days off to be with him that turned out not to be covered.
