4 different options. Paccar pays for most of it.

Company funded health care at three (3) different co-pay levels and additional "HMO" options at reasonable cost for employee and dependents. Plan provisions within "HMO" option were very accommodating to various conditions. Choices seemed to be available for "young and single" employees to "older with preventive needs" employees and in-between.

Great coverage with low deductibles (if you select option A). Very affordable

PACCAR offers a number of different Premera plans (e.g., high-deductible with low monthly premium or low-deductible with higher monthly premium). plus a Group Health option. The monthly premiums are pretty reasonable. The dental and vision plans are pretty good -- 100% of preventative dental care is covered and children can get a free pair of glasses every year.

the price is OK, but the coverage is not the best

It is very basic and nothing is special about it. The dental coverage is even more limited.
