Blue Cross/Blue Shield, good coverage, but with all health plans, the cost has risen consistantly

It is very expensive compared to other companies and the choices are being stripped away every year.

Best: Blue Cross/Blue Shield, automatically has claims submitted against flex spending balance.
Worst: Relatively expensive, and going up in price every year (e.g. $1,000 family deductible for PPO account).
