High co-payments and you are required to go through a physical to qualify for benefits. If you meet all their health requirements, you get the lowest rate otherwise the weekly rates go up. Who has perfect health - weight, bp,sugar,bmi,chelestrol, smoker/non smoker.

The best thing, you have health benefits. The worst thing is that it is becoming less affordable each year especially for families.
