Company pays 100% premium for employee & dependents. They switched to high-deductible plan in 2016 though... Out-of-pocket deductible is now $3-4K. I miss my old plan! Still better than most.

Really high deductible which has changed from previous years. However, they offer a $600 contribution incentive if you complete a wellness check onsite within a certain time window.

Has some deductible amount which is good for beginners, but not quite helpful once you have a family

Used to have one of the best plan (zero deductible) before 2016. Now you need to pay $4500 each year before the insurance company kicked in.
