In my state (AZ), it is UHC and you can chose between PPO or HSA as well as basic and Choice. We carry PPO Choice and have 80% coverage, low deductible ($350 PP). I had several health issues in a previous year and met my out of pocket maximum of $2,000 (PP). Premiums do rise every year, but coverage is decent. IVF is covered as well as meds and other fertility procedures. I have had better insurance coverage through the same carrier, but only when working in pharmaceuticals. Overall, the carrier isn't bad and the plan is better than any place my husband and friends have worked at locally.

Best: Low deductible and max out-of-pocket expense for best PPO plans. HSA options available too. Worst: Kinda pricey on monthly basis - you get what you pay for.

Monthly contributions, co-pays, co-insurance and deductibles continue to steadily rise while benefits continue to decrease. Overall good offerings, but each year you pay more and get less.
