PTO starts out at 3 weeks per year, then after 2 years it goes up to 4 weeks per year, at 5 years you get 5 weeks per year. Director and above get unlimited PTO.
I think the benefits are lacking. Health insurance is very mediocre, maternity leave is the bare minimum, and there's no additional benefits to make up for that.

vRad was very good with their PTO; they also respected and encouraged you to use your PTO. I felt that the work life/balance is important to the leaders. I also felt that as you became a part of the company, they really respected and took into consideration the needs of the individual.

The health plan is a joke. Premiums are high, coverage is almost nothing. Out of pocket for everything until you've paid $4-5K out of pocket.
