Applied via company website, received email from recruiter, had great phone call with recruiter, then the hiring manager flaked twice when he was supposed to call. The first time, I called the recruiter to see if there was a miscommunication. There wasn't and I was told, "Oh, he just walked by and went into a meeting, we'll have to reschedule." For the second call, I gave the hiring manager 30 minutes beyond our scheduled call time and then I called him directly. The phone rang, he didn't answer, and the inbox was full. When I emailed him, he replied quickly with, "Sent you a message that a meeting was running over, can you chat in 5?" I replied with a yes and he called 20 minutes later.
Red flags all over the place, but the bottom line is this is an incredibly cool company doing awesome things, but if the hiring manager can't even keep a calendar to interview candidates, there's often much larger issues.

There weren't any difficult questions. It was more like, "Tell me about you." Other questions were, "Would you be comfortable focusing on external communications versus internal?"
