Sue (not her real name), a recent CAPABLE graduate, is a 92-year-old woman living with neuropathy and osteoarthritis. She joined the program after experiencing frequent falls, severe pain, and increasing difficulty managing daily activities at home. During her initial visits, Sue shared concerns about safely getting in and out of her tub and high bed, as well as her inability to easily call for help after a fall. She also struggled to communicate with her primary care provider and access the equipment and medications she needed. As Sue built trust with the CAPABLE team, the occupational therapist, registered nurse, and handyperson worked together to address her goals. They helped her connect with her physician, leading to medication changes that significantly reduced her pain, and assisted her in obtaining a portable exerciser she now uses every day. The team also completed home modifications, including installing grab bars, stair railings, a bedrail, a lever door handle, and other adaptive equipment to improve her safety and independence. By the end of the program, Sue was no longer experiencing falls, reported significantly less pain, felt safer moving throughout her home, and achieved four of her six personal goals—allowing her to live more confidently, independently, and comfortably at home....Show more

The spirit of giving was on full display this holiday season thanks to the generosity of employees from Care Synergy, The Denver Hospice, and Colorado Visiting Nurse Association.
