Description Longevity Health Plan, in clinical partnership with LiveWell Choice, is a new Medicare Advantage Institutional Special Needs Health Plan (ISNP) specifically available for long term nursing home residents. Essential to the success of our Company is the patient centered nurse practitioner clinical model.
We offer a competitive benefit package which includes medical, dental, and vision insurance with the employee contributions to the plan being pre-tax. At this time the Company contributes 90% of the cost of the plan contributions for employees and 70% for their family members.
In addition to these pre-tax benefits, we also provide a $10,000 term life insurance policy at no cost to the employee that will be in force for the duration of their employment. We also offer the ability to enroll in voluntary term life insurance in addition to what the Company provides with the ability to enroll a spouse and dependents, as well as short and long-term disability plans.
We also offer a HSA or FSA, a pre-tax commuter benefit program, an Employee Assistance Program (EAP), Pet insurance, Auto and Home insurance, Legal insurance, Accident insurance, Critical Illness insurance, Hospital Indemnity insurance, and a 401(k) plan. Our PEO, TriNet, also offers many employee discount programs which are too numerous to list.
We also provide paid continuing educations days and a stipend to pay for some or all of the continuing education costs. In addition to the continuing education days, we provide for up to 160 hours of paid time off each year, 3 paid bereavement leave days, 5 paid jury duty days, 9 paid legal holidays, and 1 floating holiday which may be taken on a day of the employees choosing.
We also offer a Company sponsored Learning Management System which, in addition to the mandatory training courses, offers an array of courses for professional growth at no cost to the employee. These courses can help to position employees for growth and advancement within the Company.

We began our quest into senior living because of our love for a beloved family member, Mary Bickford, whose needs became too great for us to manage. After Mary was diagnosed with Alzheimer's, Judie and her husband Don Eby decided to create a home for her, inspired by her life of love, service and happiness. Because of her life, we saw the other "Mary's" out there and dedicated our lives to reaching out to them.
