Primary Care Partners is a well-established primary care facility that has been in the Grand Valley for over 25 years. Formally established in 1998, PCP is made up of 9 physician-owned practices partnering together to offer a team approach to healthcare. Our highly-trained providers are deeply embedded in the community and serve over 55,000 patients throughout western Colorado and eastern Utah.
We offer family medicine, physical, occupational, and speech therapy, pediatric care, diagnostics and mammography, nutrition therapy and diabetes education, care coordination, and after-hours care.
