For 50 years, Chapa-De Indian Health has been providing exceptional care to our Northern California communities. The "Chapa-De Culture" is built around a connection between our staff and patients. Chapa-De operates non-profit community health centers in Auburn and Grass Valley, California. We are also building a new health center in Rocklin, California that will open in late 2026. Chapa-De health centers provide whole-person, patient centered healthcare. Chapa-De offers adult and pediatric primary medical care, prenatal care, nutrition and health education, diabetes prevention and wellness programs, maternal child health program, dental care, orthodontia, mental health counseling, substance use disorder services, psychiatry, optometry, on-site pharmacies, care coordination and more.
