The Mar 27th review about middle management couldn't be more spot on. Middle and Senior management will not accept that they are the problem. They are constantly meddling with procedures, reorganizations every quarter, they can't quite figure out why they don't see results.
They are so wrapped up in playing politics with each other and keeping their gravy train afloat they ignore the company burning around them. They're also not technical at all so they are unable to distinguish qualified and unqualified employees. This results in some engineers and architects that barely qualify as 2nd rate sysadmins.
The politics from management and unqualified engineers feeds a culture of defensiveness, blame, and "not my job" syndrome. This has resulted in crumbling and barely functional infrastructure that is constantly crashing and/or misconfigured and as long as they can blame someone else there's no desire to fix.
The flagship product is not only difficult to maintain and troubleshoot but R&D and Cloud Operations are so busy trying to one up each other and play games that they don't have any time left over to create a product that actually works.
