Poor Management. Incompetent leadership. Measured by if your liked or not, not by technical ability.

I was on the Small Agency Accounts Team, with prem less than $20K. Management never appreciated all we did, as these accounts are expensive to maintain and do not produce a great deal of premium. They laid off our manager after she worked there 10 yrs doing a fabulous job, and split her responsibilities among Personal Lines Managers, with no Comm Lines experience, and a few corporate VP's. The youngest person on the team was promoted to Team Lead, though her insurance experience was all of 2 yrs. It was clear who they wanted to groom for long-term. No pay increases over the 3.5 yrs I was there.
