Management and several federal civilian staff demonstrated a consistent lack of understanding of their own policies and processes. Definitions of key terms and procedures were unclear or contradictory, and employees were often expected to follow processes that leadership themselves could not accurately explain. In some cases, staff were instructed that a process worked one way, only to later be told the opposite, creating confusion and risk for those trying to comply in good faith. There was also a culture of retaliation and reputation damage when concerns were raised about potential waste, fraud, or abuse. Employees who attempted to raise compliance or accountability issues were discouraged and, in some cases, informally undermined by colleagues instead of supported through proper reporting channels. This environment made it difficult to do the job correctly, raised compliance concerns, and contributed to a lack of trust between staff and leadership.
