Pros

-Mindful Therapy Group manages all credentialing with insurance and billing for providers -they offer a lot of admin support services -associate level therapists can make $70+ an hour which is a hard opportunity to find when you are not fully licensed

Cons

-pay for licensed therapists is not great due to the 40% “commission” MTG takes. I don’t use any of their services other than credentialing and billing and so the 40% is a pretty steep cut. I average $75-$78 a session with MTG, but at Headway I average $121 per session (with a lot less demand on me and access to better support tools). My private pay clients (I use Simple Practice pay $150 for individual and $200 for couples). Once the interstate counseling and social work licensing compacts go live in 2026, MTG will have an eve harder time competing with companies like Headway because I can be licensed in 14+ states and Headway will credential me with the relevant regional insurance companies and handle billing, while reimbursing me at an hourly rate that is $40-$50 higher an hour than at MTG. That will likely be when I leave MTG. -If you’re a clinical supervisor over associates, the demands and liability for your is very high and the reimbursement does not at all match what is required of you, especially after the recently cut supervisor reimbursement rate by more than 50%. MTG makes a ton of money if licensed supervisors who may have up to 15 associates. That means one licensed provider with 15 associates and their own caseload may be bringing in revenue for 240 clients a week (average of 15 clients per provider), which at 40% commission = $11,000k+ per week for MTG off of one provider. And that’s on the low end. They need licensed providers or they wouldn’t be able to have the large volume of associates, which is the bulk of their contracted providers, thus they’d have astronomically lower client volumes. Licensed providers keep the lights on at MTG and should’ve reimbursed better for it -their reimbursement system is unnecessarily messy and hard to track. They have a minuscule sliver of the market compared to much bigger companies like Headway, so they probably don’t have the cash flow to pay providers like Headway does but at Headway, I am paid 2 weeks after each session I complete, before insurance has even paid Headway. At MTG you’re not paid until insurance pays out, so don’t expect a paycheck for 8-12 weeks when you first start. It’s better once you’re in a regular payment cycle but that takes months as you’re building your caseload.

Thank you for sharing such detailed feedback. We take provider perspectives seriously and appreciate the opportunity to offer additional context for those considering different practice models. Mindful Therapy Group has supported independent clinicians for over 15 years and currently works with more than 2,300 providers across six states. Our model is intentionally designed as a bundled support structure for independent practices, including insurance credentialing and enrollment, billing and reimbursement operations, client placement coordination, compliance and regulatory support, administrative onboarding, negotiated group reimbursement rates, access to both in-person and telehealth opportunities, and many other supportive pieces. While not every service is used equally by every provider, the structure reflects the full operational and compliance infrastructure required to support large, multi-state clinical practices. Compensation structures, reimbursement timing, and supervision models vary widely across the industry. National marketplaces, EAPs, and group practice infrastructures each operate under different financial, regulatory, and risk assumptions. In particular, clinical supervision involves significant oversight, compliance obligations, and liability considerations that extend beyond session volume alone. These factors meaningfully shape how supervision is structured and reimbursed within any compliant care model. We also recognize that expectations around reimbursement speed and compensation have shifted as new platforms enter the market. At the same time, Mindful has successfully adapted through multiple industry changes over the past decade and continues to evaluate how our model evolves alongside licensing compacts, payer requirements, and provider needs. Many clinicians who build consistent caseloads with Mindful choose to remain long-term because the infrastructure and support align with their professional goals. Others determine that a different model is a better fit, and we respect those decisions. Practice models are not one-size-fits-all. If your experience did not align with your expectations, we welcome direct conversation to better understand what felt misaligned and to ensure future providers have clear, transparent information when choosing the practice environment that best supports them. We appreciate this feedback and remain committed to transparency, thoughtful evolution, and supporting sustainable, compliant clinical practices.

Pros

Super transparent about the entire process including costs, caseload ramp up, Mindful expectations and limitations right from the beginning! I have never felt pressured, micromanaged or taken advantage of in any way and in fact it is the opposite! I’ve felt like I have so much support, excellent supervision opportunities and got paid faster than expected too! ;) Every single human I’ve interacted with who is affiliated with MTG has been genuinely lovely. I’m so thankful for the opportunity to partner with MTG and find better work/life balance!

Cons

Not really a “con” for MTG but just private practice for an associate in general is to be prepared for a lower take home percentage until you are fully licensed. TRUST ME though, what you are “paying” is so worth it for the support you get.
